LifeBox is the digital pre-operative assessment tool used by The Montefiore Hospital.
If you are coming in to our hospital for surgery, you will need a pre-operative assessment. We use a digital pre-operative assessment tool called Lifebox to prepare you for your procedure.
Using this digital tool means a more personalised pre-operative assessment experience by identifying your needs earlier, a reduction in the need for face-to-face visits to the hospital, and keeps the information you need in one convenient place.
An account will be created for you by a member of the hospital team, and they will require your email address and a mobile telephone number. If you do not have an email address or mobile telephone number we can offer you an assisted digital service; please ask one of the hospital team about this.
Once an account has been created for you, you will need to activate and login to the account. You can do so via any device with a web browser.
You will then need to complete a series of questions about your lifestyle and circumstances, medical and surgical history and current health and medications. A condition of your admission to The Montefiore Hospital is that you meet our pre-operative assessment requirements. You must also answer all questions honestly and provide as much information as possible, so that we can assure your safety. Members of the clinical team will access your information to assist them in assessing your suitability for surgery and anaesthesia.
LifeBox also assists in preparing you for admission through the provision of information about your surgery and anaesthetic.

The information held in your LifeBox account belongs to you, but your use of LifeBox permits The Montefiore Hospital to access this information for the purpose of pre-operative assessment.
Pre-operative assessment is undertaken in accordance with national, evidence-based and best-practice guidelines and the ultimate aim is to ensure your safety and wellbeing.
Someone from the Admissions Team will contact you with a pre-operative assessment appointment. You will be asked to complete the questions prior to this appointment. This appointment is usually done as a telephone call, but if you need to attend the hospital, we will let you know. A number of factors are taken into account in determining which type of appointment is most appropriate for you.
You can access assistance by using the LifeBox intercom, which is the blue icon in the bottom left of the screen. Activating your account
You will receive an email from LifeBox with an activation link. Please check your junk/spam folders if you have not received this email within 24 hours, and let the hospital know (see contact details overleaf).
Use the activation link to access the sign-up page. You will need to create a secure password and confirm this password, then click ‘sign-up’.
Once you have signed up you will see the confirmation page. A text message will be sent to your mobile phone, containing a passcode which you will need to enter here along with your email address, then click ‘confirm’.
You will now see the login page. Please enter the password you have just created, then click ‘open LifeBox’.
Once you have logged into your LifeBox account you will see body system icons in a circle that we refer to as the ‘wheel’. You must click on each body system and answer the questions presented.
Depending on how you answer each question you may be asked further questions. You will notice that when you have completed all required questions relating to a body system the icon on the wheel will change to a green tick.
You do not have to answer all questions in one attempt but we will not be able to view your information until all questions have been answered.
Please answer all questions as soon as possible (but no later than 48 hours after registration) so that we can progress your pre-operative assessment. Your surgery may need to be deferred if you do not comply with this request.
Depending on the type of surgery you are having, you may be presented with some additional questions relating to the effect of your current condition on aspects of your daily life. This information is required to help your surgeon evaluate the outcome of surgery at a later time.
You are strongly advised to watch the information videos that are made available to you in LifeBox. This is to ensure that you are fully informed about the planned surgical procedure and anaesthetic.
Once you have watched the video as you will be asked to select one of the following statements:
I have watched and understood the content
I have watched and I have questions.
If you have questions about the video content the assessment nurse will discuss this with you.
- Your medical history - preferably from last 5 years.
- Your medication history
- Some measurements, including your height and weight. If you need to, putting in estimates is okay, but you will be formally measured at a later stage.
Your Stage 2 pre-operative assessment can go ahead as long as the survey is completed any time before your allotted appointment. We will send a reminder text 72 hours before this appointment, and will call you the day before if you have not yet completed your survey.
You can use Lifebox on any device with a web browser.
However, for optimum usage, we recommend using a laptop/computer and Google Chrome.
The majority of patients take between 15-30 minutes to complete their questionnaires, with only a small fraction taking over an hour.
You do not need to complete the survey in one sitting. Your progress is saved as you complete the different sections, so you can return to the survey if you need to take a break or get more information.
